Overview Since our doors opened in 1968 CHI Health Plainview has been providing patients of Pierce Antelope and Knox counties in Nebraska with exceptional care and quality outcomes.
We are a 15-bed critical access facility with inpatient and outpatient services.

A contributes to the mission and vision of CHI Health by performing personal care and household management tasks, utilizing approrpriate infection control techniques.
Follows a written plan of care developed by a registered nurse, for patients in their homes or other places of residence.
The incumbent reports to the supervisor of the home health team.
The incumbent works collaboratively with the other staff members to achieve the goals of CHI Health and the home care department.
The home health aide receives supervision and direction regarding the patient's plan of care.
Travel to all locations is required.
Responsibilities Has the authority to interview, hire, orient, terminate, promote, train and conduct performance evaluations.
Assigns day-to-day work activities; directs the functional and technical job performance of team members.
Allocate and direct staffing needs to meet patient, unit, and department needs, and collaborate with others to ensure overall staffing needs are met.
Manage performance including:
setting goals, clarifying job expectations, monitoring performance progress, providing feedback, recognize, developing skills and addressing performance issues related to work and our Commitments using corrective action.
Participates in all safety, security and infection control programs that are mandatory as well as those required and provided by the department.
Demonstrates work practices consistent with CHI Health and department specific safety, security and infection control practices.
Demonstrates the CHI Health commitments in daily behaviors.
Values and embraces multicultural traditions and recognizes when there is a need for and procures translations/interpretations appropriately.
Complies with all internal and extrenal regulations, policies and proceedures.
Demonstrates understanding and practice of standards and HIPAA regulations.
Supports home care efforts for continuous quality improvement.
Maintains training and educational requirements of organization and licensure.
Participates in defining home care stragegic priorities and in achieving its goals and thus contributes to planning for use of physicial, human and capital budget fiscal resources.
Provides ideas, comments and recommendations to home care through established forums, team meetings, steering groups, leadership communications and in any opportunity for open dialogue.
Performs clinical proceedures which do not require the skill/knowledge or judgement of a licensed professional and which may be safely delegated in accordance with home care policies, procedures and state professional licesnce requirements.
Provides personal care to the assigned patients under the supervision of a registered nurse.
Performs these activities as assigned:
Assists patient with bathing, skin care, nail care (except as medically contraindicated), oral hygiene and dressing.
Assists with toileting and elimination, including:
Incontience care Care of an indwelling catheter Administration of retention or cleansing enema Bowel and bladder training Observing and reporting patterns of elimination Recording intake and output Collection of speciments (urine, stool, sputum) Emptying of ostomy appliance or changing the bag Performs service procedures and treatments within scope of practice of the state and per home care lisensure and policy requirements.
Medication administration is not within scope for home care and practice is in compliance with the Nebraska Medication Act.
Measures and records vital signs, including blood pressure, pulse, respirations, and temperature as directed.
Assists with appropriate activity, including positioning, ambulation, transfers, range of motion (both active and passive), use of assistive devices, and therapeutic exercises as instructed to the patient by the professional personnel.
Application of non-sterile dressings to superficial wounds, and application of heat or cold as directed.
Performs incidental household tasks which are essential to the patient's well-being and safety, including:
Changing linens Laundry necessary to meet patient's needs.
Keeps kitchen, bathroom, and patient care area clean and tidy.
Observes the patient to determine changes in condition or in family situation; reports findings to the registered nurse.
Promotes patient's well-being by providing basic emotional and psychological support to the patient and other members of the household.
Recognizes and reports safety hazards and obstacles in the patient care environment.
Utilizes safe techniques when providing care, promotiong well-being of the patient, family and self.
Recognizes the patient's rights including the need for respect of patient and family property, and the right to privacy.
Abides by the home care policies and proceedures regarding confidentiality.
Follows established home health aide plan of care and documents interventions and related observations of the clinical record according to established policies and proceedures.
If utilizing and electronic device, communicates daily and maintains the devices per home care standards.
Assists with nutritional needs, according to the plan of care, such as:
Planning and preparing nutritious meals.
Assisting or feeding the patient as required.
Performs and demonstrates the ability to respond to basic emergency situations, utilizing the emergency response system when indicated.
Participates in patient orientated team conferences, contributing to the plan of care.
Carries personal auto insurance coverage per CHI Health policies.
Authorized to make enteries in the medical record, consistent with and limited to the scope of the job duties and any applicable lisensure.
Must have the ability to work effectively and collaboratively in a diverse and multi-cultural environment.
Welcomes other duties and special projects as assigned.
Qualifications Must have a current, valid driver's license, and valid auto insurance.
Current BLS.
Active on State of Nebraska C.

The fraudster will send a check to the victim who has accepted a job. The check can be for multiple reasons such as signing bonus, supplies, etc. The victim will be instructed to deposit the check and use the money for any of these reasons and then instructed to send the remaining funds to the fraudster. The check will bounce and the victim is left responsible.
